can my medical records be sent to my primary care?

Yes, you will be required to sign a medical release form and we can send your medical records at your request.

can i be a patient if you are not my primary care?

Yes, we encourage you to use our services for common illness. We can diagnosis, and treat common illnesses that keep us from being our best. We ask that you fill out our forms including past medical history and medications so we can provide the best care, that is catered specifically to you.

what if i lose my DOT card, or paperwork for my job?

A 25$ fee will be added for all lost documents that require time to be taken away from other patients to duplicate your documents.

what does a determination pending mean for a dot physical

This is very common, and means more information is needed before a determination can be made. Most commonly, documents are needed such as compliance documents for sleep apnea, stress test results, or hemoglobin A1C for diabetes monitoring. If you have ANY questions regarding what you should bring to your visit, please contact us and we can help better prepare you for your visit. By bringing the proper documents to your visits, this will facilitate getting your card much quicker.

What is needed for a sports physical?

Bring documents completely filled out prior to your appointment. This will help make your appointment much faster. If your child wears glasses make sure these are present. If any daily medications are needed for serious conditions seizures, diabetes, or heart related issues bring a note from their primary care.

Can you electronically send in medications?

Yes, all medications excluding controlled substances can be sent in to your pharmacy of choice.

Can you provide x-ray?

Yes, however you will be sent to an imaging facility. We do not have the capability to perform xray due to the nature of our business.
